Relationship & Rate Info & Approved Charges Fields

Can you make these required also? The cardholder should have to pick what their relationship is, but more importantly, the approved charges need to be chosen or they shouldn't be able to advance thru, given that this is a form for payment.

Further, when using a mobile device to fill out this form, the bubbles (checkmark boxes) to select the charge types look blacked out to me, so people aren't using these options like they used to.

Additionally, we’ve made some visual improvements to how checkboxes and radio buttons appear on both the unsigned and signed versions of the document.
